Management of Diabetes in Patients with CKD Stage 5
Introduction to CKD Stage 5 Management
- The American Diabetes Association recommends that patients with type 2 diabetes and CKD stage 5 require careful management of their glucose levels to prevent hypoglycemia and other complications 2
- Patients with substantial decreases in eGFR have a 5-fold increase in severe hypoglycemia frequency when using glucose-lowering agents 1
Glycemic Control in CKD Stage 5
- Patients with type 2 diabetes and CKD stage 5 require a 50% reduction in total daily insulin dose, demonstrating how severely kidney failure affects glucose metabolism 2
- The Kidney Disease: Improving Global Outcomes (KDIGO) guidelines recommend using SGLT2 inhibitors as first-line therapy for patients with type 2 diabetes and CKD, with an eGFR ≥30 mL/min/1.73 m² (Grade 1A) 3, 4
- For patients with CKD stage 5, SGLT2 inhibitors can be continued even if eGFR falls below 30 mL/min/1.73 m² for kidney and cardiovascular protection 4, 6
Alternative Therapies for CKD Stage 5
- Long-acting GLP-1 receptor agonists are recommended when metformin and SGLT2 inhibitors cannot be used or glycemic targets are not met (Grade 1B) 4, 6
- Dulaglutide can be used with an eGFR >15 mL/min/1.73 m² without dose adjustment 4, 6
- If sulfonylurea must be used, second-generation sulfonylureas like glipizide and gliclazide are preferred over first-generation agents because they lack active metabolites 1
Monitoring Requirements for CKD Stage 5
- HbA1c is not recommended for CKD stage 5 on dialysis due to decreased accuracy from anemia and shortened red cell lifespan 2, 1
- Continuous glucose monitoring (CGM) or frequent self-monitoring of blood glucose is recommended to prevent hypoglycemia 2, 1
- Monitor eGFR every 3-6 months at minimum in CKD stage 4-5 7
Critical Pitfalls to Avoid in CKD Stage 5 Management
- Never use first-generation sulfonylureas in any degree of renal impairment—these must be completely avoided 1
- Avoid glyburide entirely in CKD—it is contraindicated 2, 5
- Do not rely on HbA1c alone for glycemic monitoring in dialysis patients 2, 1
- Temporarily discontinue or reduce sulfonylurea doses during acute illness, surgery, prolonged fasting, or critical medical illness when hypoglycemia risk is heightened 4, 1
