Atrial Fibrillation Management with Cough Suppressants

Safety Profile

  • Dextromethorphan, the active ingredient in Robitussin, does not interact with atrial fibrillation management or worsen the arrhythmia, and is not listed among drugs that induce or exacerbate atrial fibrillation, according to the American College of Cardiology 1
  • The American College of Cardiology guidelines do not identify cough suppressants as contraindicated medications in patients with atrial fibrillation, with a strength of evidence based on comprehensive reviews 2, 3, 4
  • Dextromethorphan does not prolong the QT interval, does not affect AV nodal conduction, and does not interfere with rate or rhythm control strategies, as recommended by the American College of Cardiology 5

Considerations with Concurrent AF Medications

  • Dextromethorphan has no known interactions with beta-blockers, calcium channel blockers, or digoxin, which are first-line agents for ventricular rate control in AF, according to the American College of Cardiology and Circulation guidelines 3, 4
  • There are no documented interactions between dextromethorphan and antiarrhythmic drugs, including flecainide, propafenone, sotalol, dofetilide, or amiodarone, as stated by the American College of Cardiology 2, 5, 4
  • Dextromethorphan does not affect warfarin metabolism or direct oral anticoagulants, so no adjustment in anticoagulation monitoring is required, as recommended by Circulation 4

Important Caveats

  • Patients with atrial fibrillation should avoid combination products containing multiple ingredients beyond dextromethorphan, and check the label to ensure the product contains only dextromethorphan, as advised by Pharmacological Research 1
  • Decongestants, such as pseudoephedrine or phenylephrine, should be avoided as they can trigger atrial fibrillation or increase ventricular rate, according to Pharmacological Research 1
