Management of Seizures in Uremic Encephalopathy

Primary Treatment Approach

  • The American College of Emergency Physicians recommends benzodiazepines, such as lorazepam or diazepam IV, for active seizures to terminate the acute event, following standard status epilepticus protocols, in patients with uremic encephalopathy 1

Role of Antiepileptic Drugs

  • The American Academy of Neurology suggests that additional AEDs, such as levetiracetam, phenytoin, or valproate, may be administered for refractory status epilepticus in patients with uremic seizures 1
  • The American Academy of Neurology recommends that prophylactic AEDs should NOT be routinely prescribed after a first uremic seizure once the metabolic derangement is corrected, with evidence showing that antiepileptic drugs should not be routinely prescribed to adults after a first unprovoked seizure 2

Duration of AED Therapy

  • The American Academy of Neurology suggests that discontinuation of AED treatment should be considered after 2 seizure-free years in patients who required ongoing therapy, with a strength of evidence grade of moderate 2
  • The American Academy of Neurology recommends that for patients with persistent seizures despite adequate dialysis, AEDs should be continued long-term and standard epilepsy management guidelines should be followed, with reassessment after 2 seizure-free years 2

AED Selection in Renal Failure

  • The National Kidney Foundation recommends levetiracetam as the preferred first-line AED in patients with renal impairment due to its favorable tolerability profile and predictable pharmacokinetics, though dose adjustment is required 3
  • The National Kidney Foundation suggests that phenytoin, carbamazepine, and phenobarbital should be avoided when possible due to significant cognitive impairment, neuropsychiatric effects, and altered protein binding in uremia 3
