Recurrent Middle Ear Infections with TM Perforations in Healthy Adults

Primary Considerations for Bilateral Sequential Disease

  • The American Academy of Otolaryngology-Head and Neck Surgery suggests that Eustachian tube dysfunction is a primary consideration for bilateral sequential disease, which can be caused by allergic rhinitis, chronic rhinosinusitis, or anatomic variants that impair middle ear ventilation and pressure equalization 3
  • The presence of viral upper respiratory infections can trigger Eustachian tube dysfunction, leading to acute otitis media (AOM), with rates of AOM following URTI reaching 37% in susceptible individuals 1
  • The innate immune system of the middle ear, including mucociliary clearance, lysozyme, defensins, and complement factors, normally maintains a sterile environment, but subtle deficiencies in these defense mechanisms can predispose to recurrent middle ear disease when challenged by viral URTIs 1

Critical Diagnostic Workup Required

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends otoscopic examination of both ears to assess current TM status, looking for healed perforations, retraction pockets, or persistent effusion that would indicate ongoing Eustachian tube dysfunction 3, 2
  • Pneumatic otoscopy should be performed on the unaffected ear to assess TM mobility, with reduced mobility suggesting subclinical middle ear effusion 1, 2
  • Tympanometry of both ears provides objective evidence of middle ear status and Eustachian tube function, according to the Clinical Infectious Diseases guidelines 5

Management Strategy

  • The Praxis Medical Insights guidelines recommend using non-ototoxic topical preparations, such as ofloxacin 0.3% or ciprofloxacin-dexamethasone, if perforation is present, and avoiding aminoglycosides that cause permanent sensorineural hearing loss 6, 4
  • Systemic antibiotics targeting S. pneumoniae, H. influenzae, and M. catarrhalis should be prescribed, with high-dose amoxicillin-clavulanate as first-line or respiratory fluoroquinolone if recent antibiotic use, as suggested by the Praxis Medical Insights guidelines 6, 4
  • The Nature Reviews Disease Primers guidelines suggest considering prophylactic measures during viral URTIs, including aggressive nasal decongestants and avoiding forceful nose-blowing, to prevent future episodes of recurrent middle ear infections 1

Prevention of Future Episodes

  • The Praxis Medical Insights guidelines recommend keeping ears strictly dry by using earplugs during showering and avoiding swimming until TM healing is confirmed and underlying cause addressed, to prevent future episodes of recurrent middle ear infections 6
  • Treating any underlying allergic rhinitis or chronic rhinosinusitis aggressively with intranasal corticosteroids and antihistamines can optimize Eustachian tube function, according to the Otolaryngology-Head and Neck Surgery guidelines 3

Follow-Up and Specialist Referral

  • The Otolaryngology-Head and Neck Surgery guidelines recommend considering tympanometry at 3-month intervals to monitor for subclinical middle ear effusion that would indicate ongoing Eustachian tube dysfunction 3, 5
  • The Nature Reviews Disease Primers guidelines suggest that immunodeficiency should be considered if a third episode occurs, warranting immunologic workup including immunoglobulin levels 1
