Management of Acute Myeloid Leukemia
Initial Treatment
- For newly diagnosed AML patients under 60 years, the American Society of Hematology recommends initiating intensive induction chemotherapy with 7 days of cytarabine plus 3 days of an anthracycline, achieving complete remission in >85% of younger patients, followed by risk-stratified consolidation therapy including high-dose cytarabine or allogeneic stem cell transplantation based on cytogenetic risk 1, 2
- The European LeukemiaNet recommends cytogenetic analysis of 20 metaphase cells to identify prognostic chromosomal abnormalities 1
- Cardiac assessment with echocardiography is recommended for patients with cardiac risk factors, as anthracycline cumulative doses >300 mg/m² carry significant cardiotoxicity risk 2
Risk Stratification
- The National Comprehensive Cancer Network recommends risk stratification based on cytogenetic and molecular markers, with favorable risk including inv(16), t(16;16), or t(8;21) without KIT mutations 1, 3
- Intermediate risk includes normal cytogenetics, NPM1 mutations without FLT3-ITD 3
- High risk includes complex karyotype, monosomal karyotype, TP53 mutations, FLT3-ITD with high allelic ratio 3
Induction Therapy
- The American College of Physicians recommends standard "7+3" induction consisting of cytarabine 100-200 mg/m² continuous infusion for 7 days and daunorubicin 60-90 mg/m² or idarubicin 12 mg/m² on days 1-3 1, 2
Consolidation Therapy
- For favorable-risk patients under 60 years, the European Society for Medical Oncology recommends 3-4 cycles of high-dose cytarabine 3 g/m² every 12 hours on days 1, 3, and 5 1, 2
- For intermediate-risk patients, the American Society of Hematology recommends myeloablative allogeneic stem cell transplantation from HLA-compatible sibling in first complete remission 1
Management of Relapsed/Refractory AML
- The European LeukemiaNet recommends prognostic assessment using validated scoring, with favorable risk having 70% 1-year survival and 46% 5-year survival 3
- Salvage chemotherapy regimens include high-dose cytarabine 3 g/m² every 12 hours on days 1, 3, 5, 7 plus daunorubicin 50 mg/m² or idarubicin 10 mg/m² on days 2, 4, 6 3, 7
Supportive Care
- The National Comprehensive Cancer Network recommends prophylaxis and management of tumor lysis syndrome, antimicrobial prophylaxis during neutropenia, and red blood cell and platelet transfusions to maintain hemoglobin >7-8 g/dL and platelets >10,000/μL 2, 9
Follow-Up
- The American Society of Hematology recommends monitoring schedule for patients in remission, including bone marrow morphology every 3 months for 24 months and differential blood counts every 3 months for 5 years 2
