Using Lexapro (Escitalopram) and Amitriptyline Together

Risks of Combination Therapy

  • The combination of escitalopram and amitriptyline increases the risk of serotonin syndrome due to their overlapping serotonergic effects 1, 2
  • Serotonin syndrome symptoms can develop within 24-48 hours after combining medications and include mental status changes, neuromuscular hyperactivity, and autonomic hyperactivity 2
  • Advanced symptoms may include fever, seizures, arrhythmias, and unconsciousness, which can be life-threatening 3

Recommendations for Safe Use

  • If combination therapy is necessary, start the second medication at a low dose and increase slowly 2
  • Monitor closely for symptoms of serotonin syndrome, especially during the first 24-48 hours after dosage changes 2, 3
  • Hospital-based treatment with discontinuation of all serotonergic agents and supportive care is required if serotonin syndrome develops 2

Clinical Considerations

  • Escitalopram has less effect on CYP450 isoenzymes compared to other SSRIs, potentially making it a safer choice when combining with other medications 2

Monitoring Requirements

  • Regular monitoring is essential when using this combination 2
  • Consider more frequent follow-up visits during the initial combination period and after any dose adjustments 4

Contraindications

  • This combination is contraindicated in patients taking MAOIs or within 14 days of stopping an MAOI 1
  • Avoid in patients with cardiac conduction abnormalities or QT prolongation 2, 5

Safe Use of Amitriptyline and Escitalopram

Cardiac Considerations

  • The European Society of Cardiology recommends avoiding the combination of amitriptyline and escitalopram in patients with cardiac conduction abnormalities or QT prolongation, as amitriptyline can prolong QT interval and delay AV-node conduction, with tricyclic antidepressants like amitriptyline associated with increased risk of cardiac arrest (OR = 1.69), particularly in older patients 6
  • The European Heart Association suggests obtaining a baseline ECG before starting the combination, especially in patients over 60 years or with cardiac risk factors, due to the potential for cardiac conduction effects 6

Therapeutic Drug Monitoring

  • The International Association for Therapeutic Drug Monitoring and Clinical Toxicology recommends considering measurement of amitriptyline and nortriptyline plasma levels, particularly if side effects emerge or therapeutic response is inadequate, with a therapeutic range for amitriptyline + nortriptyline combined of approximately 80-200 ng/mL 7
