Antibiotic Treatment Guidelines

Patient Assessment and Preparation

  • The American Academy of Dermatology and the Centers for Disease Control and Prevention recommend confirming patient age is over 8 years old, assessing for hypersensitivity to tetracyclines, pregnancy status, and potential drug interactions before initiating doxycycline treatment, and considering baseline STI screening for gonorrhea, chlamydia, and syphilis, as well as baseline blood tests for tickborne diseases if clinically indicated 1, 2, 3, 4

Medication Administration and Monitoring

  • The American Academy of Dermatology, the Centers for Disease Control and Prevention, and the Thorax guidelines recommend taking doxycycline with adequate fluids, such as a full glass of water, and with food to minimize gastrointestinal side effects, separating the dose by at least 2 hours from dairy products, antacids, and supplements containing calcium, iron, magnesium, or sodium bicarbonate, and monitoring for adverse effects such as gastrointestinal symptoms, skin reactions, and photosensitivity reactions 1, 2, 5
  • Monitor for treatment response and evaluate clinical improvement based on the condition being treated, with follow-up testing 3-6 months after treatment for sexually transmitted infections, and consider follow-up serology 2-4 weeks after treatment for tickborne diseases 2, 3, 4

Adverse Effects and Contraindications

  • The American Academy of Dermatology, the Centers for Disease Control and Prevention, and the Thorax guidelines recommend monitoring for adverse effects such as gastrointestinal symptoms, skin reactions, and photosensitivity reactions, and warn against prescribing doxycycline to pregnant women, children under 8 years, and patients with certain medical conditions, such as hepatic impairment and systemic lupus erythematosus 1, 2, 5
  • Common adverse effects of doxycycline include gastrointestinal symptoms such as nausea, vomiting, diarrhea, and dysphagia, as well as dermatological symptoms such as photosensitivity and rash, while less common but serious adverse effects include exfoliative dermatitis, erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis, esophagitis, and esophageal ulcerations 5
  • Patients should take precautions to minimize the risk of photosensitivity reactions, such as wearing protective clothing, seeking shade, and avoiding direct sunlight, especially around midday, and be aware that UV radiation can be stronger when reflected by water, sand, or snow 2, 6
  • Moderate or occasional alcohol consumption does not significantly affect doxycycline levels, but patients with chronic alcoholism may require adjusted dosing to maintain therapeutic levels, as recommended by the Thorax guidelines 5

Special Considerations

  • Doxycycline is contraindicated in children under 8 years due to risk of permanent teeth discoloration, and during pregnancy and breastfeeding, as recommended by the Thorax guidelines 5
  • Patients with certain medical conditions, such as hepatic impairment and systemic lupus erythematosus, should use doxycycline with caution, as recommended by the Thorax guidelines 5
