Medicare Documentation Requirements for Home Health Therapy and Walker

Walker Prescription Requirements
- The American College of Physicians recommends that the prescription must specify the type of walker based on patient needs: standard walker, two-wheeled walker, or four-wheeled walker, and document specific functional limitations that necessitate the walker 2, 4
Medical Justification Criteria
- The Centers for Medicare and Medicaid Services requires documentation of at least one qualifying condition, such as a patient diagnosed with a new disease or has multiple comorbidities, unstable or fragile medical status, or functional limitations requiring assistance with ADLs or IADLs 1
Home Environment and Safety Assessment
- The American Academy of Physical Medicine and Rehabilitation recommends documenting home safety evaluation findings and recording environmental modifications needed, such as handrails, adequate lighting, and removal of loose rugs 1, 4
Caregiver and Social Support Documentation
- The National Institute on Aging recommends identifying and listing all available caregivers in the medical record, documenting caregiver capacity to meet care needs and any limitations, and recording whether the patient is living alone or has inadequate support systems 1
