Treatment for Partial Central Diabetes Insipidus

Diagnosis Confirmation

  • Measure serum sodium, urine volume, and osmolality to establish baseline values, and plasma copeptin levels <21.4 pmol/l should prompt testing for AVP deficiency (central DI), according to the Nature Reviews Nephrology guidelines 1
  • Genetic testing may help distinguish central DI from nephrogenic DI, especially in cases with partial insensitivity to AVP, as recommended by the Nature Reviews Nephrology guidelines 2
  • Response to desmopressin administration is diagnostic for central DI (versus nephrogenic DI which shows no response), as stated in the Nature Reviews Nephrology guidelines 3

Treatment Protocol

Fluid Management

  • Allow ad libitum access to fluid to prevent dehydration, and patients capable of self-regulating should rely on their thirst sensation rather than prescribed fluid amounts, as suggested by the Nature Reviews Nephrology guidelines 4

Monitoring and Follow-up

  • Regular monitoring of serum electrolytes (sodium, potassium, chloride), urine volume and osmolality, and body weight is crucial, according to the Nature Reviews Nephrology guidelines 5
  • Ultrasound of the urinary tract every 2-3 years to detect potential complications, as recommended by the Nature Reviews Nephrology guidelines 5
