COVID-19 Treatment Guidelines

Introduction to COVID-19 Treatment

  • The American College of Physicians recommends treatment for COVID-19 to be stratified by disease severity and oxygen requirement, with dexamethasone 6 mg daily for 10 days as the cornerstone therapy for all hospitalized patients requiring supplemental oxygen, anticoagulation for all hospitalized patients, and antiviral therapy (nirmatrelvir/ritonavir or remdesivir) initiated within 5-7 days of symptom onset for high-risk outpatients or hospitalized patients not on mechanical ventilation 1, 2

Treatment Algorithm by Clinical Severity

  • For high-risk outpatients with mild COVID-19, the American Medical Association recommends initiating antiviral therapy as soon as possible after diagnosis and within 5 days of symptom onset, with first-line options being nirmatrelvir/ritonavir (Paxlovid) or molnupiravir 1
  • The American College of Cardiology recommends dexamethasone 6 mg daily for 10 days for hospitalized patients requiring supplemental oxygen, which reduces mortality by 3% 1, 2
  • The Infectious Diseases Society of America recommends prophylactic-dose anticoagulation for all hospitalized patients, with low molecular weight heparin (LMWH) preferred over unfractionated heparin 2
  • For patients with severe COVID-19, the American Thoracic Society recommends prone positioning for patients receiving invasive ventilation, which reduces mortality 2

Special Populations

  • The National Comprehensive Cancer Network recommends pre-exposure prophylaxis with long-acting anti-SARS-CoV-2 monoclonal antibodies for unvaccinated or high-risk immunocompromised patients 1
  • The American Society of Hematology recommends post-exposure prophylaxis with anti-SARS-CoV-2 monoclonal antibodies for high-risk individuals not expected to mount adequate immune response 1

Critical Pitfalls to Avoid

  • The American College of Physicians recommends against using corticosteroids in patients not requiring oxygen, as it causes harm without benefit 1, 2
  • The American Thoracic Society recommends against using remdesivir in mechanically ventilated patients, as it has no survival benefit 2
  • The Infectious Diseases Society of America recommends against giving tocilizumab without corticosteroids, as the mortality benefit requires concomitant corticosteroid therapy 3

Adjunctive Care

  • The American Physical Therapy Association recommends rehabilitation care to be implemented as soon as oxygenation and hemodynamics are stable 1
  • The American Psychological Association recommends psychological support for patients experiencing anxiety, fear, or depression 1, 2
