Management of Uncontrolled Hypertension

Assessment and Treatment

  • The patient's blood pressure of 174/94 mmHg indicates uncontrolled Grade 2 hypertension, requiring immediate treatment intensification 3
  • The patient is already on an ACE inhibitor (lisinopril 20mg) but has not achieved target blood pressure, suggesting the need for combination therapy 4
  • The American College of Cardiology recommends adding a dihydropyridine calcium channel blocker (DHP-CCB) to the current regimen for patients with uncontrolled hypertension on lisinopril 20mg daily 1, 2
  • The European Society of Cardiology guidelines support the use of a DHP-CCB as the preferred next agent for patients with uncontrolled hypertension on ACE inhibitor therapy 6

Treatment Algorithm

  • Verify medication adherence to lisinopril 20mg daily 5
  • Ensure proper blood pressure measurement technique (using validated device with appropriate cuff size) 1
  • Implement or reinforce lifestyle modifications (sodium restriction, weight loss, physical activity, alcohol moderation) 2
  • Add a DHP-CCB as the preferred next agent, which is recommended by guidelines as an effective two-drug regimen 1, 6
  • Consider using a single-pill combination when available to improve adherence 6

Monitoring and Follow-up

  • Reassess blood pressure within 2-4 weeks after treatment modification 7
  • Target blood pressure should be <130/80 mmHg 9
  • Aim to achieve target blood pressure within 3 months of treatment initiation 10

Important Considerations

  • Fixed-dose combination pills improve adherence and should be preferred when available 6
  • Avoid combining two RAS blockers (such as adding an ARB to the current ACE inhibitor) 6
  • Consider potential drug interactions and comorbidities when selecting additional agents 5
