Paracetamol Dosing Guidelines for Adolescents

Standard Dosing Guidelines

  • The American Society of Anesthesiologists recommends a maximum of 6 doses in 24 hours, not exceeding 4000 mg total daily, with a minimum interval of 4 hours between doses, for adolescents 12 years and older 1
  • The European Society for Paediatric Anaesthesiology guidelines support the use of a standard oral dose of 10-15 mg/kg every 6 hours, with a maximum of 60 mg/kg/day, for weight-based dosing in adolescents 1

Route-Specific Considerations

  • For intravenous administration, a loading dose of 15-20 mg/kg, followed by maintenance of 10-15 mg/kg every 6-8 hours, with a maximum daily dose of 60 mg/kg or 4000 mg, whichever is lower, is recommended for adolescents 1

Critical Safety Warnings

  • The risk of hepatotoxicity increases significantly with any dose exceeding 4000 mg in 24 hours, and chronic exposures greater than 140 mg/kg/day for several days, especially with concurrent alcohol use 2
  • The American College of Medical Toxicology recommends reducing doses and extending intervals in patients with hepatic impairment, and considering limiting to 2-3 grams/day maximum in patients with chronic alcohol consumption 2

Alternative Treatments

  • The European Society for Paediatric Anaesthesiology guidelines recommend considering ibuprofen (10 mg/kg every 8 hours) as second-line treatment if paracetamol is ineffective, with the potential for combination therapy to provide additional fever-free time 1, 2
