Guideline for Initiating and Titrating Venlafaxine When Switching from Duloxetine

Initiation Dose

  • Begin venlafaxine at 37.5 mg once or twice daily after completing the duloxetine taper. (Mayo Clinic Proceedings) 1

Titration Schedule

  • Increase the venlafaxine dose by 75 mg each week as tolerated, aiming for a therapeutic range of 150–225 mg per day. (Mayo Clinic Proceedings) [1][2]
  • Expect 2–4 weeks to reach an efficacious dosage within the therapeutic range. (Mayo Clinic Proceedings) [1][2]
  • Do not exceed a maximum dose of 225 mg per day. (Mayo Clinic Proceedings) 1

Cardiovascular Monitoring During Initiation

  • Monitor blood pressure and pulse regularly because venlafaxine can cause sustained hypertension, elevated blood pressure, and increased pulse. (American Academy of Child and Adolescent Psychiatry) [3][5]
  • Include height, weight, pulse, and blood pressure measurements in routine monitoring. (American Academy of Child and Adolescent Psychiatry) [3][5]
  • Be aware that a small number of patients have reported cardiac conduction abnormalities. (Mayo Clinic Proceedings) [1][2]

Alternative Cross‑Taper Strategy (When a medication‑free interval is not tolerated)

  • Reduce duloxetine to 20 mg daily while simultaneously starting venlafaxine 37.5 mg daily. (Mayo Clinic Proceedings) 1
  • After one week, discontinue duloxetine and continue increasing venlafaxine according to the standard titration schedule. (Mayo Clinic Proceedings) 6

Discontinuation Recommendations for Both SNRIs

  • Implement a slow taper when discontinuing either duloxetine or venlafaxine to prevent withdrawal syndromes. (Mayo Clinic Proceedings; American Academy of Child and Adolescent Psychiatry) [6][4]5
  • Specifically, venlafaxine should be tapered when treatment ends because a withdrawal syndrome has been described. (Mayo Clinic Proceedings) [1][6]2
  • SNRIs, like SSRIs, require a gradual discontinuation taper to minimize adverse events. (American Academy of Child and Adolescent Psychiatry) [3][4]5

Safety Monitoring for Suicidality (Patients ≤ 24 years)

  • Assess for suicidal thoughts and behaviors within 1–2 weeks of any medication change. (Annals of Internal Medicine) 9
  • Watch for agitation, irritability, or unusual behavioral changes as potential warning signs. (Annals of Internal Medicine) 9
