Azithromycin Dosing Regimens for Various Infections

Evidence-Based Dosing Regimens

  • The UK guidelines recommend azithromycin 12mg/kg/day (maximum 500mg once daily) for 5 days when treating non-pharyngeal Group A Streptococcal colonization, particularly when penicillin treatment alone has failed 1, 2, 3
  • Azithromycin 500mg daily for 3 days in three cycles with 7-day intervals has shown efficacy in treating blepharitis, though an alternative pulsed regimen of 1g weekly for 3 weeks is also acceptable 6
  • The standard 5-day regimen with dose reduction after day 1 is more typical for treating severe infections or immunocompromised hosts, though extending therapy to 7 days may be necessary 7
  • A 5-day continuous 500mg regimen (total 2.5g) provides a higher total dose than standard regimens and is recommended for difficult-to-eradicate carriage sites where standard therapy has failed 1, 2, 3
  • The FDA issued a warning in 2013 regarding QT prolongation and serious cardiac arrhythmias associated with azithromycin use 6
  • Perform ECG before initiating therapy and avoid azithromycin if QTc >450ms (men) or >470ms (women) 5
  • Do not administer azithromycin with aluminum- or magnesium-containing antacids as they reduce absorption by 24% 7

Clinical Decision Algorithm

  • For non-pharyngeal GAS carriage (vaginal, anal, skin), use a 5-day regimen: 500mg once daily for 5 days 1, 2, 3
  • For community-acquired pneumonia, a standard regimen is 500mg day 1, then 250mg daily for days 2-5 4, 5
  • Monitor for treatment failure in GAS carriage eradication and perform clearance screens at 24 hours, then 1, 3, 6, and 12 weeks post-treatment 3
