Irbesartan Dosing Guidelines

Special Populations and Dose Adjustments

  • The American College of Cardiology and American Heart Association recommend ACE inhibitors as first-line treatment for chronic kidney disease with albuminuria, with ARBs like irbesartan as reasonable alternatives if ACE inhibitors are not tolerated 1, 2

Monitoring and Safety Recommendations

  • When initiating or titrating irbesartan, particularly in patients with CKD, check serum creatinine and potassium within 2-4 weeks of starting therapy or dose increases, and monitor blood pressure to ensure adequate control while avoiding hypotension 3
  • Continue irbesartan therapy unless serum creatinine rises by more than 30% within 4 weeks, symptomatic hypotension develops, or uncontrolled hyperkalemia occurs despite medical management, as recommended by the American Journal of Kidney Diseases 3

Important Clinical Considerations

  • Avoid combining irbesartan with ACE inhibitors or direct renin inhibitors, as this increases the risk of hyperkalemia and renal dysfunction without additional benefit, according to guidelines from a reputable medical source, excluding Praxis Medical Insights 1, 2
