Acyclovir Dosing and Renal Management for Herpes Zoster in Elderly Patients

Renal Assessment and Monitoring

  • Renal function assessment is mandatory before initiating acyclovir in octogenarian patients because age‑related decline in glomerular filtration is nearly universal, and failure to adjust dose can lead to neurotoxicity. 1
  • During acyclovir therapy, renal function should be monitored frequently in older adults to detect early signs of nephrotoxicity and adjust dosing as needed. [1][3]

Dose Adjustments for Renal Impairment

  • In patients receiving hemodialysis, the recommended regimen is acyclovir 800 mg every 12 hours with an additional 800 mg dose administered after each dialysis session. 2

Inadequate Low‑Dose Regimens for Varicella‑Zoster Virus

  • Low‑dose acyclovir regimens (200–400 mg) that are effective for genital herpes are insufficient for herpes zoster because varicella‑zoster virus is less sensitive to acyclovir than herpes simplex virus. 3

Alternative Antivirals with Improved Bioavailability

  • Valacyclovir (1000 mg three times daily) and famciclovir (500 mg three times daily) provide higher oral bioavailability and more convenient dosing schedules for the treatment of herpes zoster. 4
  • Both valacyclovir and famciclovir require renal dose adjustment in elderly patients to avoid toxicity. 5
