Amlodipine Dosing Considerations for Elderly Patients

Initial Dosing and Titration

  • The European Society of Cardiology recommends that initial doses and subsequent titration of amlodipine should be more gradual in elderly patients due to a greater risk of adverse effects, particularly in very old and frail subjects 1, 2

Blood Pressure Targets and Monitoring

  • The American College of Cardiology suggests that for debilitated or frail persons ≥80 years of age, a systolic blood pressure goal <150 mmHg is reasonable 3, 4
  • Target blood pressure is <140/90 mmHg for most elderly patients, with individualization based on frailty status 5

Special Considerations

  • Calcium channel blockers like amlodipine cause greater antihypertensive effects in elderly patients due to decreased baroreceptor response and age-related increases in drug exposure [@ sometime after administration 1, 2

Safety Monitoring and Adverse Effects

  • The European Society of Cardiology notes that peripheral edema, orthostatic hypotension, dizziness, headache, flushing, and falls are more common in elderly patients taking amlodipine, and that monitoring for these adverse effects is necessary 1, 2

Amlodipine Dosing Considerations in Elderly Patients

Special Considerations for Elderly Patients

  • The European Society of Hypertension recommends that initial doses and subsequent titration should be more gradual in elderly patients due to greater risk of adverse effects, particularly in very old and frail subjects 6, 7
  • Monitor for orthostatic hypotension by measuring blood pressure in both sitting and standing positions, as elderly patients are at increased risk 8

Dosing Flexibility and Administration

  • Amlodipine can be administered at any convenient time of day to improve adherence, though morning dosing may confer a slight advantage 9

Common Pitfalls to Avoid

  • Avoid combining amlodipine with beta-blockers initially without careful monitoring, as they may act synergistically to depress cardiac function 10
  • Peripheral edema is the most common side effect, and starting at 2.5 mg helps minimize this 9

Amlodipine Therapy Considerations

Cardiac Function Cautions

  • The American Heart Association suggests that amlodipine appeared safe in patients with severe heart failure with reduced ejection fraction, unlike non-dihydropyridine calcium channel blockers 11
  • Non-dihydropyridine calcium channel blockers should be avoided in heart failure with reduced ejection fraction due to negative inotropic effects, but amlodipine does not share this concern 11
