Levofloxacin Administration Guidelines

Formulations and Administration Options

  • The Centers for Disease Control and Prevention recommends levofloxacin is available in multiple tablet strengths (250 mg, 500 mg, 750 mg) to accommodate different dosing requirements 1
  • An aqueous solution for intravenous administration is available as an alternative for patients who cannot swallow tablets, as recommended by the Centers for Disease Control and Prevention 1

Important Considerations for Administration

  • The American College of Clinical Pharmacy recommends that levofloxacin should not be administered within 2 hours of medications containing divalent cations (such as antacids) as these significantly decrease absorption of the fluoroquinolone 1, 2
  • The National Kidney Foundation guidelines state that the drug is primarily cleared by the kidney (80%), requiring dosage adjustments for patients with renal insufficiency 1, 3
  • For patients with creatinine clearance less than 50 ml/minute, the recommended adjusted dose is 750-1000 mg three times a week, according to the Centers for Disease Control and Prevention 1, 3
