Neutropenia in Systemic Lupus Erythematosus

Primary Mechanisms

  • High-dose glucocorticoids, cyclophosphamide, mycophenolate mofetil, azathioprine, and methotrexate all impair bone marrow function and neutrophil production, according to the Autoimmunity Reviews 1, 2
  • Abnormal host immune factors, including low complement levels, functional asplenia, and abnormal neutrophil response to pathogens, contribute to neutropenia, as reported by the Autoimmunity Reviews 1, 3

Severity Patterns

  • Severe neutropenia (<500 cells/mm³) represents a critical threshold requiring continuous monitoring for infection risk, as stated in the Annals of the Rheumatic Diseases 4, 5
