Meniscal Tear Management
Diagnostic Approach
- The American Academy of Orthopaedic Surgeons recommends joint line tenderness, knee effusion, McMurray test, Apley test, and "bounce home" maneuver as key physical examination findings for meniscal tear diagnosis 1
- The British Medical Journal suggests that limited range of motion, particularly inability to fully extend, is a significant finding in meniscal tear diagnosis 2, 3
- Radiographs should be obtained to assess for osteoarthritis, as recommended by the American Academy of Orthopaedic Surgeons 1, 5
Treatment Algorithm
- The British Medical Journal states that arthroscopic surgery is NOT recommended for degenerative knee disease with meniscal tears, as it provides less than 15% probability of small or very small improvement in short-term pain and function that does not persist to one year 2, 3
- The American Academy of Orthopaedic Surgeons recommends ACL reconstruction with autograft or allograft for young patients with ACL tears and meniscal pathology 1
- Praxis Medical Insights suggests that total knee arthroplasty is the preferred option for advanced medial compartment osteoarthritis with root tear 5
- Unicompartmental knee arthroplasty may be considered for isolated medial compartment OA in younger patients, though total knee arthroplasty has lower revision rates, as recommended by Praxis Medical Insights 5
Post-Surgical Rehabilitation
- Praxis Medical Insights recommends early mobilization to reduce pain and improve function, as well as structured physical therapy focusing on quadriceps and hamstring strengthening 4, 5
- Avoid complete immobilization to prevent muscular atrophy, as suggested by Praxis Medical Insights 4
- NSAIDs and cryotherapy (ice through wet towel for 10-minute periods) can be used for pain and swelling, as recommended by Praxis Medical Insights 4
Recovery Timeline
- Recovery typically takes 2-6 weeks, as stated by the British Medical Journal and Praxis Medical Insights 2, 3, 4
- Unable to weight bear for 2-7 days, and return to sedentary work within 1-2 weeks, as recommended by the British Medical Journal and Praxis Medical Insights 2, 3, 6
- Return to physical labor at least 6 weeks, as suggested by the British Medical Journal and Praxis Medical Insights 2, 3, 6
Special Considerations
- The British Medical Journal states that the only clear indication for arthroscopy in degenerative knee disease is patients who are objectively unable to fully extend their knee (true locked knee) 2, 3
- Praxis Medical Insights suggests that conservative management is generally not recommended as first-line treatment for bucket handle tears in young patients 4
