Deworming Considerations in Febrile Infants

Evaluation and Management of Fever

  • Infants under 1 year, particularly those under 3 months, have a relatively immature immune system that places them at significantly higher risk for serious bacterial infections (SBIs) including bacteremia, urinary tract infection, and meningitis 1, 2
  • The American Academy of Pediatrics requires full sepsis evaluation including blood culture, urinalysis via catheterization, and lumbar puncture with cerebrospinal fluid analysis for infants 0-60 days old, followed by immediate hospitalization and empiric antibiotics 1, 3
  • For infants 2-12 months old, urinalysis via catheterization is required to rule out UTI, and assessment for signs of pneumonia may require chest radiography 2, 5
  • Clinical appearance alone cannot reliably exclude serious infection—even well-appearing infants can harbor bacteremia or early meningitis 1, 3

Practical Clinical Algorithm

  • The American Academy of Pediatrics emphasizes that serious bacterial infections require prompt identification and treatment to reduce morbidity and mortality, and thus deworming should be deferred until the infant is afebrile and stable 1, 2
  • A practical clinical algorithm involves documenting rectal temperature, assessing for toxic appearance, and obtaining appropriate cultures and studies based on age, followed by initiation of empiric antibiotics if indicated, and close follow-up 1, 2, 3, 5

Diagnostic Considerations

  • Urinary tract infections account for over 90% of serious bacterial infections in febrile children under 2 years of age, with E. coli being the leading pathogen 4
  • Catheterization has 95% sensitivity and 99% specificity compared to 26% contamination rates with bag collection for urine specimen diagnosis 3, 5
