Apixaban Dosing for New Venous Thromboembolism

Treatment Dosing Regimen

  • The American College of Cardiology recommends apixaban 10 mg orally twice daily for the first 7 days, then reduced to 5 mg twice daily for continued treatment in patients with new venous thrombosis (DVT or PE) 1
  • For patients with creatinine clearance (CrCl) greater than 30 mL/min, the standard dosing of apixaban (10 mg BID × 7 days, then 5 mg BID) can be used without adjustment 1

Renal Function Considerations

  • In patients with CrCl between 15-30 mL/min, there is no FDA-approved recommendation for VTE treatment with apixaban, and alternative anticoagulation should be considered 1
  • Apixaban is not recommended for VTE treatment in patients with CrCl less than 15 mL/min or those on dialysis 1

Duration of Treatment

  • For provoked VTE (surgery, trauma, temporary risk factor), a minimum of 3 months of treatment with apixaban is recommended 1
  • For unprovoked VTE or ongoing risk factors, extended therapy should be considered, and after 6 months, the dose may be reduced to 2.5 mg BID for secondary prevention 1
  • In cancer-associated VTE, the National Comprehensive Cancer Network (NCCN) guidelines recommend low-molecular-weight heparin (LMWH) as first-line treatment, though apixaban is an acceptable alternative 1

Special Circumstances

  • If surgery is planned within 3 months of VTE diagnosis, elective procedures should be deferred until the initial treatment course is completed when possible 1
  • If a patient develops renal dysfunction during treatment, renal function should be reassessed regularly, and if CrCl drops below 30 mL/min, transition to warfarin may be necessary 1

Apixaban Dosing Regimens

Standard Dosing Regimens

  • The American College of Cardiology recommends starting apixaban at the appropriate dose based on patient characteristics, with no titration or loading phase needed, for patients with atrial fibrillation 2
  • For venous thromboembolism treatment, the standard regimen is 10 mg orally twice daily for the first 7 days, followed by 5 mg orally twice daily, which is the standard therapeutic approach, not a loading dose 3, 4
  • The National Comprehensive Cancer Network recommends that apixaban can be started immediately as monotherapy, without the need for bridging with parenteral anticoagulation, unlike dabigatran and edoxaban 3, 4
  • For VTE treatment, the 10 mg twice daily for 7 days is the standard regimen, and failure to use this higher initial dose may compromise efficacy, according to the National Comprehensive Cancer Network 3, 4

Key Considerations

  • The National Comprehensive Cancer Network advises against using parenteral anticoagulation as a "bridge" when starting apixaban for atrial fibrillation or VTE treatment, as it increases bleeding risk without benefit 3, 4
