Cardiac Biomarker Measurement
Introduction to Cardiac Biomarkers
- Cardiac troponins T and I are highly specific markers for myocardial damage, exclusively expressed in cardiac myocytes, as stated by the European Society of Cardiology 1
- Troponin has nearly absolute myocardial tissue specificity, reflecting even microscopic zones of myocardial necrosis, as reported by the American College of Cardiology 2
- CK is less tissue-specific than cardiac troponin, found in both cardiac and skeletal muscle, as noted by the American College of Cardiology 2
Measurement Guidelines for CK and Troponin
- The American Heart Association recommends initial CK measurement at presentation, with repeat measurements at 6-9 hours later, and an additional sample between 12-24 hours if clinical suspicion remains high 3
- For suspected reinfarction, the American Heart Association suggests immediate measurement followed by repeat in 3-6 hours 3
- A 20% change in CK values should be considered significant when evaluating trends, according to the American Heart Association 3
- Troponin elevation occurs in myocardial infarction, non-ischemic myocardial injury, such as myocarditis, severe congestive heart failure, pulmonary embolism, and cardiotoxic chemotherapeutic agents, as reported by the European Society of Cardiology 1
- Elevations beyond 5-fold the upper reference limit have high (>90%) positive predictive value for acute type 1 MI, according to the European Society of Cardiology guidelines, while elevations up to 3-fold the upper reference limit have limited (50-60%) positive predictive value for MI 4
Characteristics of Troponin and CPK-MB
| Characteristic | Troponin | CPK-MB |
|---|---|---|
| Cardiac specificity | Higher | Lower |
| Sensitivity for small MI | Higher | Lower |
| Duration of elevation | 4-14 days | 24-36 hours |
| Risk stratification value | Superior | Good but less powerful |
| Ability to detect reinfarction | Limited | Better |
Clinical Interpretation and Management
- Even minor elevations of cardiac biomarkers can indicate increased risk and warrant thorough evaluation, as stated by the American Heart Association 8
- Timing matters: troponin may remain elevated for 7-14 days after myocardial infarction, while CK typically returns to normal within 2-3 days, as stated by the American Heart Association 6
- Obtain 12-lead ECG to evaluate for ischemic changes, assess for chest pain, shortness of breath, or other cardiac symptoms, and review vital signs for hemodynamic stability, as part of the initial assessment 9
- Echocardiography is recommended to assess for regional wall motion abnormalities and evaluate left ventricular function, and consider CT angiography if pulmonary embolism is suspected, especially if RV/LV ratio ≥1.0, as recommended by the European Society of Cardiology 10, 11, 12
- Consider coronary angiography and possible revascularization (PCI or CABG) based on findings in patients with elevated cardiac biomarkers and positive biomarkers, as suggested by the American Heart Association 13
Special Considerations
- Black individuals have higher baseline CK levels than South Asian or white individuals, and men exhibit higher CK activities following exercise compared to women, although the relationship between CK levels and actual muscle membrane damage is complex and not fully understood 14
- Statin-associated myopathy and inflammatory myopathy are other potential causes of elevated CK levels, as noted by the American College of Cardiology and the Mayo Clinic 15, 16
- Measure troponin I at the start and every 3 weeks during trastuzumab therapy, along with echocardiography and global longitudinal strain (GLS) at the start and every 3 months, to monitor cardiotoxicity, as recommended by the American Society of Clinical Oncology and the American College of Cardiology 17, 18
- Hold immunotherapy for moderate-severe cases of skeletal muscle injury and consider permanent discontinuation with myocardial involvement, as recommended by the American Society of Clinical Oncology 19, 20
Monitoring and Follow-up
- Routine CK monitoring in asymptomatic patients on medications like statins is not universally recommended, according to the American College of Cardiology 15
- A mild elevation in CK levels, particularly after exercise, rarely indicates significant pathology requiring intervention, as noted by the American College of Sports Medicine 14
- The American College of Sports Medicine recommends determining if recent exercise, especially unaccustomed or eccentric exercise, could explain the elevation in CK levels 14
- For exercise-related elevation, the American College of Sports Medicine suggests allowing adequate recovery time, as CK typically returns to baseline within 24-120 hours with rest 14
