Oliguria Causes and Management
Pre-Renal Causes
- Oliguria results from three broad pathophysiologic categories: pre-renal (inadequate renal perfusion), intrinsic renal (kidney parenchymal injury), and post-renal (urinary tract obstruction), with pre-renal causes being the most common and reversible if identified early 1
- Pre-renal oliguria represents an appropriate physiologic response to decreased effective circulating volume or renal blood flow rather than true kidney injury 2
- Hemorrhage (trauma, gastrointestinal bleeding, surgical blood loss) can cause pre-renal oliguria 3
- Gastrointestinal losses (vomiting, diarrhea, nasogastric suction) can lead to pre-renal oliguria 4
- Renal losses (diuretic overuse, osmotic diuresis, diabetes insipidus) can cause pre-renal oliguria 4
- Third-spacing (burns, pancreatitis, sepsis, hypoalbuminemia) can result in pre-renal oliguria 4
- Inadequate fluid intake with ongoing insensible losses can lead to pre-renal oliguria 4
- Cardiac tamponade or massive pulmonary embolism can cause pre-renal oliguria 4
- Severe arrhythmias compromising cardiac output can result in pre-renal oliguria 4
- Anaphylaxis with profound vasodilation can cause pre-renal oliguria 4
- Neurogenic shock following spinal cord injury can lead to pre-renal oliguria 4
- Hepatorenal syndrome in advanced liver disease can cause pre-renal oliguria 4
- Medications (NSAIDs, ACE inhibitors, ARBs, calcineurin inhibitors) can cause renal vasoconstriction leading to pre-renal oliguria 5
- Hypercalcemia can cause renal vasoconstriction leading to pre-renal oliguria 6
Intrinsic Renal Causes
- Nephrotoxic ATN from medications (aminoglycosides, amphotericin B, contrast agents, cisplatin) can cause intrinsic renal oliguria 5
- Pigment nephropathy (rhabdomyolysis with myoglobin, hemolysis with hemoglobin) can cause intrinsic renal oliguria 6
- Drug-induced AIN (antibiotics, NSAIDs, proton pump inhibitors, diuretics) can cause intrinsic renal oliguria 5
- Infection-related AIN (pyelonephritis, viral infections) can cause intrinsic renal oliguria 5
- Autoimmune disorders (lupus, Sjögren's syndrome) can cause intrinsic renal oliguria 5
- Rapidly progressive glomerulonephritis (ANCA-associated, anti-GBM disease, immune complex) can cause intrinsic renal oliguria 5
- Severe pre-eclampsia/HELLP syndrome with glomerular endotheliosis can cause intrinsic renal oliguria 3
- Renal artery thrombosis or embolism can cause intrinsic renal oliguria 4
- Renal vein thrombosis can cause intrinsic renal oliguria 4
- Thrombotic microangiopathy (HUS, TTP) can cause intrinsic renal oliguria 5
- Atheroembolic disease following vascular procedures can cause intrinsic renal oliguria 5
- Uric acid crystallization in tumor lysis syndrome can cause acute oliguric renal failure 6
- Calcium phosphate deposition in the renal interstitium and tubules with hyperphosphatemia can cause intrinsic renal oliguria 6
- Oxalate crystals (ethylene glycol toxicity) can cause intrinsic renal oliguria 5
- Drug crystals (acyclovir, methotrexate, sulfonamides) can cause intrinsic renal oliguria 5
Post-Renal Causes
- Bilateral ureteral obstruction from retroperitoneal fibrosis, malignancy, or stones can cause post-renal oliguria 4
- Obstructive uropathy contraindicated for diuretic use can cause post-renal oliguria 6
- Bladder outlet obstruction (benign prostatic hyperplasia, prostate cancer, urethral stricture) can cause post-renal oliguria 4
- Neurogenic bladder with urinary retention can cause post-renal oliguria 4
- Blocked urinary catheter (must be excluded first when assessing oliguria) can cause post-renal oliguria 4
Special Clinical Contexts
- Inadequate volume resuscitation during surgery can cause oliguria 3
- Excessive blood loss requiring transfusion can cause oliguria 3
- Anesthetic effects on renal perfusion can cause oliguria 3
- Abdominal compartment syndrome causing renal vein compression can cause oliguria 4
- Severe pre-eclampsia progressing to acute tubular necrosis or cortical necrosis can cause oliguria 3
- HELLP syndrome with hemolysis, elevated liver enzymes, and thrombocytopenia can cause oliguria 3
- Postpartum hemorrhage with hypovolemic shock can cause oliguria 3
Important Clinical Caveats
- Oliguria may be appropriate as a normal physiologic response to volume depletion requiring resuscitation rather than indicating AKI 2
- Weight-based calculations are problematic in obesity, and adjusted body weight should be considered 2, 1
- Diuretics can confound interpretation of oliguria and should not be used to "treat" oliguria without first addressing the underlying cause 2
