Management of Chronic Urticaria Progressing to Swelling Despite Prednisone

Introduction to Chronic Urticaria Management

  • The British Journal of Dermatology recommends distinguishing between chronic spontaneous urticaria with angioedema and hereditary angioedema, as these require different treatments 2, 3
  • A low C4 level has high sensitivity for C1 inhibitor deficiency, and serum C4 should be ordered immediately to rule out hereditary angioedema 3

Critical First Step: Rule Out Hereditary Angioedema

  • The British Journal of Dermatology suggests that if C4 is low, confirmation with quantitative and functional C1 inhibitor assays is necessary 3
  • ACE inhibitors must be avoided in patients with angioedema without wheals and used with caution when angioedema accompanies urticaria 3

Why Prednisone Is Failing and Should Be Stopped

  • The British Journal of Dermatology recommends restricting oral corticosteroids to short courses only for severe acute urticaria or life-threatening angioedema affecting the mouth 2
  • Prolonged corticosteroid use leads to significant adverse effects without addressing the underlying pathophysiology, according to Praxis Medical Insights 1

Immediate Management Algorithm

  • Praxis Medical Insights recommends starting with up to 4 times the standard daily dose of a second-generation antihistamine, and continuing for 2-4 weeks before determining inadequate response 1
  • Omalizumab 300mg subcutaneously every 4 weeks is the next step if there is inadequate control after 2-4 weeks, according to Praxis Medical Insights 1
  • Allow up to 6 months for patients to demonstrate a response to omalizumab before considering it a failure, as suggested by Praxis Medical Insights 1

Special Consideration: Monitoring Disease Control

  • The Journal of Allergy and Clinical Immunology and Praxis Medical Insights recommend using the Urticaria Control Test (UCT) to objectively assess disease control, and the Angioedema Control Test (AECT) for monitoring angioedema 4, 1
  • Once complete symptom control is achieved, maintain the effective dose for at least 3 consecutive months before attempting step-down, according to Praxis Medical Insights 1

Critical Pitfalls to Avoid

  • The British Journal of Dermatology and Praxis Medical Insights advise against using prolonged corticosteroids as maintenance therapy for chronic urticaria, as this exposes patients to significant morbidity without addressing the disease mechanism 2, 1
  • Avoid NSAIDs, aspirin, codeine, and alcohol as these can exacerbate chronic urticaria, according to the British Journal of Dermatology and Praxis Medical Insights 3, 1
