Understanding Cytolytic Vaginosis
- Cytolytic vaginosis involves excessive growth of hydrogen peroxide-producing lactobacilli, causing epithelial cell lysis and symptoms similar to other vaginal infections 1
- Cytolytic vaginosis is associated with an acidic vaginal pH (typically below 4.0), while bacterial vaginosis typically has an elevated pH above 4.5 1
First-Line Treatment
- Sodium bicarbonate (baking soda) douches or sitz baths to increase vaginal pH and reduce lactobacilli overgrowth 1
- Typical regimen: 30-60 g of baking soda in 1 liter of warm water used as a douche or sitz bath 2-3 times weekly for 2 weeks 1
Important Distinctions from Bacterial Vaginosis
- Bacterial vaginosis is treated with antibiotics such as metronidazole or clindamycin, as recommended by the Centers for Disease Control and Prevention 2, 3, 4
- Cytolytic vaginosis would worsen with standard bacterial vaginosis treatments, as antibiotics may further promote lactobacilli overgrowth 1
