Community-Acquired Pneumonia Management

Indications for Hospital Admission

  • The European Respiratory Society recommends hospital admission for elderly patients with community-acquired pneumonia and relevant comorbidities, such as diabetes and hypertension, due to increased risk of complications and mortality 1, 3
  • Patients with severe illness, characterized by tachypnea, tachycardia, and confusion, require hospital admission for close monitoring and treatment 1, 2

Antibiotic Therapy

  • The European Respiratory Society guidelines recommend combination therapy with a second or third generation cephalosporin, such as ceftriaxone, and a macrolide, such as azithromycin, for hospitalized pneumonia patients 3
  • Ceftriaxone provides coverage for typical bacterial pathogens, including Streptococcus pneumoniae, while azithromycin covers atypical organisms, including Mycoplasma, Chlamydia, and Legionella 3, 5, 6

Monitoring and Management

  • Clinical reassessment at 48-72 hours is necessary to evaluate fever resolution and lack of progression of pulmonary infiltrates 3, 5, 6
  • Daily evaluation of mental status, respiratory rate, oxygen requirements, and renal function is crucial for early detection of potential complications 2, 6
  • Consideration for ICU transfer is necessary if respiratory rate remains elevated, altered mental status worsens, or hemodynamic instability develops 2
