Fetal Distress Management
Critical Assessment of Fetal Heart Rate Patterns
- A Category III fetal heart rate pattern, characterized by minimal variability, late decelerations, and prolonged decelerations, indicates a high-risk situation for fetal acidosis and requires immediate attention 1, 2
- The American Academy of Family Physicians notes that a non-reactive NST with minimal variability is a sign of compromised fetal central nervous system activity 3, 4
- Late decelerations, which occur after the peak of contractions, reflect uteroplacental insufficiency and increase the risk of fetal acidosis 5, 2
- Prolonged decelerations lasting more than 2 minutes but less than 10 minutes, combined with minimal variability, significantly increase the concern for fetal acidosis 5, 4
- Variable decelerations, suggesting cord compression, can be a sign of significant fetal compromise when combined with minimal variability and late decelerations 3, 5
Immediate Management Algorithm
- The American College of Obstetricians and Gynecologists (ACOG) recommends stopping oxytocin immediately, changing maternal position to left lateral, administering oxygen at 6-10 L/min, and giving an IV fluid bolus to improve fetal oxygenation and relieve cord compression 1, 2
- Continuous monitoring of fetal heart rate tracing is crucial to assess response to resuscitative measures, and delivery should be expedited if the tracing does not rapidly normalize 1
- A cesarean section is the recommended route of delivery in cases of unfavorable cervix and concerning fetal status 1, 2
Evidence Strength
- The recommendation for expedited delivery is supported by ACOG guidelines prioritizing intervention for Category III tracings 1
- The American Academy of Family Physicians provides evidence that prolonged decelerations are associated with increased rates of nonreassuring fetal status requiring cesarean delivery 5, 4
- The Journal of the American College of Radiology notes that a normal biophysical profile (BPP) score can be falsely reassuring in cases of acute fetal compromise 6
