Sleep Disturbances in Elderly Patients

Non-Pharmacological Interventions

  • The American Geriatrics Society recommends non-pharmacological interventions as the first-line treatment for sleep disturbances in elderly patients, with cognitive behavioral therapy for insomnia (CBT-I) being the most effective approach, demonstrating sustained benefits for up to 2 years 1
  • CBT-I combines multiple evidence-based techniques, including sleep restriction/compression therapy, which limits time in bed to actual sleep time, gradually increasing as sleep efficiency improves 1
  • Stimulus control therapy, which involves going to bed only when sleepy, leaving the bedroom if unable to fall asleep within 15-20 minutes, and maintaining consistent sleep-wake times, is also effective 2

Pharmacological Options

  • The American Geriatrics Society suggests that melatonin receptor agonists be used as the first choice for pharmacological treatment, starting at the lowest available dose due to their safer profile and minimal adverse effects in elderly patients 2
  • Short-acting non-benzodiazepines (Z-drugs) can be used as a second choice, with eszopiclone and zolpidem being options, but caution is advised due to the risk of next-morning psychomotor and memory impairment 2
  • Short-acting benzodiazepines should be used with extreme caution and only when absolutely necessary, with a dose reduction of 50% of the standard adult dose and close monitoring for respiratory depression, confusion, and fall risk 5

Special Populations

  • For nursing home residents, a multicomponent approach including increased daytime sunlight exposure, structured bedtime routines, reduced time in bed during the day, and minimized nighttime noise and light interruptions is recommended 4
  • For patients with dementia or Alzheimer's disease, non-pharmacological interventions such as bright light therapy, physical activity, and structured routines should be prioritized, with melatonin receptor agonists being the first choice for pharmacological treatment if needed 3

Critical Monitoring Parameters

  • When using any sleep medication in elderly patients, critical monitoring parameters include respiratory depression, confusion or delirium, falls and fractures, next-day cognitive impairment, and worsening dementia symptoms 5

Common Pitfalls to Avoid

  • Starting with pharmacotherapy instead of non-pharmacological interventions, using standard adult doses, long-term benzodiazepine use, ignoring underlying causes, and abrupt discontinuation of medication are common pitfalls to avoid 1, 5, 6
