Management of Patients with Significant Carotid Artery Stenosis

Antiplatelet Therapy Recommendations

  • The American College of Cardiology recommends antiplatelet therapy with aspirin alone (75-325 mg daily), clopidogrel alone (75 mg daily), or the combination of aspirin plus extended-release dipyridamole for patients with symptomatic carotid stenosis (recent stroke or TIA within 6 months) 1
  • The American College of Cardiology recommends antiplatelet therapy with aspirin (75-325 mg daily) for asymptomatic patients with >70% carotid stenosis to prevent myocardial infarction and other ischemic events 2

Management Based on Symptom Status

For Symptomatic Patients

  • The American College of Cardiology and the Journal of Neurointerventional Surgery recommend initiating antiplatelet therapy immediately with clopidogrel 75 mg daily for symptomatic patients (with recent stroke/TIA) 1, 4
  • The American College of Cardiology and the Journal of Neurointerventional Surgery recommend carotid revascularization (CEA or CAS) as soon as possible, ideally within 2 weeks of the index event, for symptomatic patients (with recent stroke/TIA) 4, 5

For Asymptomatic Patients

  • The American College of Cardiology recommends starting antiplatelet therapy (clopidogrel 75 mg daily) for asymptomatic patients with >70% carotid stenosis 2
  • The International Journal of Stroke recommends optimizing medical therapy while awaiting vascular consultation for asymptomatic patients 7

Timing Considerations

  • The American College of Cardiology and the Journal of Neurointerventional Surgery recommend revascularization within 2 weeks of the index event for symptomatic patients rather than delaying surgery 1, 4
  • The Journal of Neurointerventional Surgery recommends starting clopidogrel while awaiting vascular consultation to reduce stroke risk 8

Special Considerations

  • The Journal of Neurointerventional Surgery recommends carotid endarterectomy (CEA) over carotid artery stenting (CAS) for older patients (>70 years) when revascularization is indicated 1
  • The Journal of the American College of Cardiology recommends dual antiplatelet therapy with aspirin and clopidogrel before and for a minimum of 30 days after CAS 10, 8
