Valsartan Therapy

Introduction to Valsartan

  • The American College of Cardiology recommends valsartan as a treatment option for hypertension, with a target dose of 160 mg twice daily (total daily dose 320 mg) for optimal cardiovascular outcomes 2
  • Valsartan has a dose-dependent efficacy over the range of 80-320 mg once daily for lowering blood pressure in adults and pediatric patients ≥1 year of age, reducing the risk of fatal and nonfatal cardiovascular events 1

Heart Failure with Reduced Ejection Fraction

  • The American Heart Association suggests that valsartan reduces the risk of hospitalization in patients with heart failure (NYHA class II-IV), with a 13.2% reduction in the combined endpoint of mortality and morbidity 3, 4
  • The Val-HeFT trial demonstrated that valsartan was effective in reducing heart failure hospitalizations, primarily driven by reduced heart failure hospitalizations 3, 4

Post-Myocardial Infarction

  • The American College of Cardiology recommends valsartan as a treatment option for post-myocardial infarction patients with left ventricular failure or dysfunction, with a target dose of 160 mg twice daily 2
  • The VALIANT trial demonstrated that valsartan was noninferior to captopril for reducing cardiovascular events in high-risk post-MI patients 3, 4, 6

Safety and Tolerability

  • The European Society of Cardiology notes that valsartan has a significantly lower incidence of cough compared to ACE inhibitors, and a side effect profile comparable to placebo in most studies 1, 5
  • The American College of Cardiology recommends monitoring renal function and potassium levels, especially in patients with baseline renal impairment, due to the potential risk of adverse events 1, 5

Clinical Outcomes Evidence

  • The American Heart Association suggests that the cardiovascular benefits of valsartan include significant reductions in heart failure hospitalizations, comparable magnitude of effect to ACE inhibitors for mortality and morbidity reduction, and reductions in stroke incidence in high-risk hypertensive patients 3, 4, 8
  • The European Society of Cardiology notes that valsartan improves NYHA functional class, ejection fraction, and quality of life in heart failure patients 3, 4
