Antibiotic Categories and Clinical Applications

Introduction to Antibiotic Categories

  • The American College of Clinical Microbiology recommends penicillins, such as penicillin G, amoxicillin, and amoxicillin-clavulanate, for streptococcal infections, skin infections, and respiratory tract infections 1, 2
  • The Infectious Diseases Society of America suggests cephalosporins, including first-generation (cefazolin, cefalexin), second-generation (cefuroxime), third-generation (ceftriaxone, cefotaxime), and fourth-generation (cefepime), for various infections based on their gram-positive and gram-negative coverage 3, 4, 5, 6, 7

Beta-Lactam Antibiotics

  • The European Society of Clinical Microbiology and Infectious Diseases recommends carbapenems, such as meropenem and imipenem, for severe or multi-drug resistant infections due to their broadest spectrum of activity 7, 8

Aminoglycosides and Fluoroquinolones

  • The Journal of Microbiology, Immunology and Infection suggests aminoglycosides, such as gentamicin, tobramycin, and amikacin, often in combination with beta-lactams, for serious gram-negative infections, including Pseudomonas aeruginosa 6
  • The Clinical Microbiology and Infection journal recommends fluoroquinolones, including ciprofloxacin, levofloxacin, and moxifloxacin, for respiratory, urinary, and gastrointestinal infections due to their broad-spectrum activity 5, 7

Macrolides and Tetracyclines

  • The Clinical Microbiology and Infection journal suggests macrolides, such as azithromycin, clarithromycin, and erythromycin, for respiratory infections, particularly community-acquired pneumonia, due to their effectiveness against gram-positive bacteria and atypical pathogens 5, 9
  • The Journal of Microbiology, Immunology and Infection recommends tetracyclines, including doxycycline and minocycline, for atypical infections, rickettsial diseases, and some skin infections 4, 6

Glycopeptides and Oxazolidinones

  • The Clinical Microbiology and Infection journal suggests glycopeptides, such as vancomycin and teicoplanin, for serious infections with resistant gram-positive bacteria, including MRSA 1, 2, 3
  • The Clinical Microbiology and Infection journal recommends oxazolidinones, such as linezolid, for MRSA and VRE infections due to their unique mechanism of action and effectiveness against resistant gram-positive bacteria 10

Clinical Application Based on Infection Type

  • The Clinical Microbiology and Infection journal recommends beta-lactams, macrolides, and fluoroquinolones for respiratory infections 5, 9
  • The Clinical Microbiology and Infection journal suggests beta-lactams, clindamycin, TMP-SMX, and linezolid for skin and soft tissue infections, including MRSA 1, 3, 4
  • The Clinical Microbiology and Infection journal recommends beta-lactams with beta-lactamase inhibitors, carbapenems, and combinations with metronidazole for intra-abdominal infections 7
  • The Clinical Microbiology and Infection journal suggests combination therapy with glycopeptides plus piperacillin-tazobactam or carbapenem for necrotizing fasciitis 1, 2
