Management of Cerebral Edema in Bilateral ACA Infarcts

Initial Conservative Measures

  • The American Heart Association recommends restricting free water to avoid hypo-osmolar fluids that worsen edema, using isoosmotic or hyperosmotic maintenance fluids in patients with bilateral ACA infarcts 2, 3
  • Elevating the head of bed 20-30 degrees with neck in neutral position is recommended to optimize venous drainage in patients with cerebral edema 2, 4
  • Correcting aggravating factors, including hypoxemia, hypercarbia, and hyperthermia, is crucial as these exacerbate cerebral swelling 2, 5
  • Avoiding vasodilating antihypertensives, such as nitroprusside, is recommended as they can increase intracranial pressure 4

Mannitol Use

  • The American College of Cardiology suggests that mannitol should only be administered when there are clinical signs of elevated ICP or impending herniation, such as declining level of consciousness, pupillary changes, or decerebrate posturing 1, 3
  • Mannitol is recommended at a standard dose of 0.25-0.5 g/kg IV over 20 minutes, repeated every 6 hours as needed, with a maximum daily dose of 2 g/kg 1, 2, 3
  • The onset of action of mannitol is 10-15 minutes, with a duration of 2-4 hours 3, 6

Alternative and Adjunctive Therapies

  • Hypertonic saline (3% or 23.4%) has comparable efficacy to mannitol at equiosmolar doses and is preferred when hypovolemia or hypotension is present 3
  • Decompressive hemicraniectomy performed within 48 hours reduces mortality and improves functional outcomes in large hemispheric infarcts, and is considered the most definitive treatment when medical management fails 4, 5
  • External ventricular drainage can rapidly reduce ICP if hydrocephalus is present, particularly in cases of obstructive hydrocephalus caused by bilateral ACA infarcts 4

Mortality and Outcome

  • Despite intensive medical management, including mannitol, mortality in patients with increased ICP from large infarcts is 50-70% 1, 3, 6
  • The use of mannitol does not improve outcomes in ischemic brain swelling, according to the American Heart Association 1, 4
