Prednisone Tapering Protocol
General Principles
- The American College of Rheumatology recommends tapering prednisone gradually using a dose-dependent approach: reduce by 5-10 mg weekly until reaching 10 mg/day, then slow to 1 mg every 4 weeks until discontinuation, with single morning dosing before 9 AM 1, 2
- Single daily morning dosing (before 9 AM) is strongly preferred to align with the body's natural cortisol rhythm and minimize HPA axis suppression 1
- For patients with prominent night pain while tapering below 5 mg daily, divided dosing may be considered 3, 4
Tapering Schedules
- For high-dose tapering (>30 mg/day), reduce by 5-10 mg weekly until reaching 10 mg/day, then transition to slow taper protocol 1, 2
- For medium-dose tapering (10-30 mg/day), reduce by 5 mg every 1-2 weeks until reaching 10 mg/day, then transition to slow taper protocol 1, 2
- For low-dose tapering (<10 mg/day), reduce by 1 mg every 4 weeks until discontinuation 3, 4, 1
Disease-Specific Modifications
- For polymyalgia rheumatica, initial dose is 12.5-25 mg daily, taper to 10 mg within 4-8 weeks, then reduce by 1 mg every 4 weeks until discontinuation 3, 4
- For immune-related adverse events (checkpoint inhibitors), start 0.5-1 mg/kg/day for grade 2-3, start 1-2 mg/kg/day for grade 3-4, and taper over 4-6 weeks once symptoms improve to grade ≤1 5, 6
Managing Relapse
- If symptoms recur at any point, immediately return to the previous effective dose, maintain for 4-8 weeks, then attempt slower taper 3, 4, 1, 2
Monitoring Requirements
- During tapering, monitor for disease flare and adrenal insufficiency, with follow-up schedule every 4-8 weeks during first year and every 8-12 weeks in second year 1, 2, 3, 4
Stress Dosing Requirements
- Patients require supplemental glucocorticoids during acute illness or stress, with double current prednisone dose for 3 days for minor illness, and hydrocortisone 50 mg 2-3 times daily for major stress/surgery 1, 2
