Intensive Cytarabine‑Containing Immunochemotherapy Followed by Autologous Stem Cell Transplantation for Young, Fit Mantle‑Cell Lymphoma Patients with Marrow Infiltration

Immediate Treatment Approach

  • Initiate therapy at diagnosis in all symptomatic patients or those with high tumor burden—including marrow infiltration causing pancytopenia—because delayed treatment worsens outcomes. Level III evidence. 2, 4, 5
  • Pancytopenia resulting from disease infiltration is a marker of high tumor burden and mandates immediate treatment rather than observation. Level III evidence. 6

Cytarabine‑Containing Induction Regimens

  • Cytarabine‑containing induction significantly prolongs median time to treatment failure versus non‑cytarabine regimens (P = 0.038) in a randomized trial. Level I evidence. 1, 3, 7
  • The intensive strategy that incorporates autologous stem‑cell transplantation (ASCT) yields higher overall response and survival rates in fit patients. Level I evidence. 1, 2, 8

Regimen options (all include high‑dose cytarabine):

  • Alternating R‑CHOP / R‑DHAP. Level II evidence. 1, 3
  • R‑hyper‑CVAD (alternating hyper‑fractionated cyclophosphamide, vincristine, doxorubicin, dexamethasone with high‑dose methotrexate/cytarabine); feasibility may be limited by toxicity. Level II evidence. 1, 7, 9
  • Nordic‑type protocols that incorporate high‑dose cytarabine. Level II evidence. 1, 3

  • Induction with high‑dose cytarabine alone provides insufficient response rates and should be avoided. Level I evidence. 1, 3, 7

Consolidation Strategy

  • ASCT consolidation is recommended for young, fit patients because it independently improves response and survival, regardless of rituximab use. Level I, Grade B evidence. 1, 2, 8
  • Adding total‑body irradiation (TBI) before ASCT benefits only patients who achieve a partial response; it offers no advantage for those in complete response. Level II evidence. 1, 3

Maintenance Therapy

  • Rituximab maintenance after R‑CHOP markedly improves progression‑free survival (PFS) and overall survival (OS). Level I, Grade A evidence. 1, 2, 8
  • Systematic meta‑analyses confirm that rituximab maintenance also enhances PFS across multiple studies. Level I evidence. 1, 3

High‑Risk TP53‑Mutated Disease

  • TP53 mutations are associated with poorer responses to both induction chemotherapy and ASCT. Level II evidence. 10
  • For TP53‑mutated patients, BTK‑inhibitor‑based combinations are recommended:
    • Ibrutinib plus high‑dose cytarabine‑based chemo‑immunotherapy. Level II evidence. 10
    • Zanubrutinib + obinutuzumab + venetoclax, achieving ~96 % overall response rate and ~88 % complete remission in this subgroup. Level II evidence. 10
  • Standard ASCT provides limited benefit in TP53‑mutated disease; integration of novel agents is essential. Level II evidence. 10

Supportive Care During Induction

  • Administer granulocyte‑colony stimulating factor (G‑CSF) as needed for neutropenia. Level III evidence. 6
  • Provide red‑cell and platelet transfusions for anemia and thrombocytopenia. Level III evidence. 6
  • Use antimicrobial prophylaxis during periods of neutropenia to prevent infections. Level III evidence. 5
  • Monitor closely for tumor‑lysis syndrome given the high tumor burden. Level III evidence. 6
