Pharmacologic Management of Vestibulodynia

Tricyclic Antidepressants (TCAs)

Topical Lidocaine

Evidence‑Based Recommendations for Vestibulodynia Treatment

First‑Line Physical Therapy

  • The multimodal physical‑therapy protocol should consist of weekly supervised sessions for 10 weeks, patient education about vestibulodynia, pelvic‑floor muscle exercises with biofeedback, manual‑therapy techniques, and progressive vaginal dilation. 4

Pharmacologic Therapy – Tricyclic Antidepressants (TCAs)

  • TCAs are recommended for the neuropathic‑pain component of vestibulodynia according to the MMWR recommendations (2022). 5
  • Evidence from the Annals of Oncology (2011) supports TCAs as an effective medical therapy for vestibulodynia. 6
  • TCAs should be initiated at low doses and titrated gradually to minimize anticholinergic side effects such as dizziness, somnolence, and gastrointestinal symptoms. 7

Age‑Related Contraindications

  • TCAs are potentially inappropriate for adults ≥ 65 years because anticholinergic effects increase the risk of falls, cognitive impairment, and urinary retention. 7

Medication Misapplication

  • Vestibular suppressants or drugs used for inner‑ear vestibular disorders are not indicated for vulvar vestibulodynia and should be avoided. 8
