Alginate Antacids in Gastroesophageal Reflux Disease Management

Introduction to Alginate Antacids

  • The American Gastroenterological Association recommends personalizing adjunctive pharmacotherapy to GERD phenotype, including alginate antacids for breakthrough symptoms, particularly in patients with delayed gastric emptying, as they do not worsen this condition 1

Clinical Applications of Alginate Antacids

  • Alginate antacids can be used for breakthrough postprandial and nighttime symptoms despite PPI therapy, targeting the postprandial acid pocket that PPIs don't fully suppress, in patients with GERD, including those with large hiatal hernias 1, 2, 3
  • For patients with coexistent gastroparesis, prokinetics should be used as adjunctive therapy, while alginates can still be used concurrently for reflux symptom control, without affecting gastric motility 1, 4
  • Alginate-antacid combinations are superior to antacids alone for mild reflux symptoms and can treat persistent symptoms despite acid suppression, in the general population with GERD 4

Dosing and Administration

  • Standard dosing of alginate antacids is 10-20 mL taken 3-4 times daily, particularly after meals and at bedtime, to target postprandial reflux when the acid pocket forms, and can be combined with PPIs without concern for drug interactions or worsening gastroparesis 1, 2, 3, 4
