Medical Necessity of ACDF for Cervical Radiculopathy

Required Documentation for Medical Necessity Determination

  • The American College of Neurosurgery requires documentation of signs or symptoms of neural compression, such as radiculopathy or myelopathy, associated with the levels being treated, as well as advanced imaging studies showing moderate to severe central/lateral recess or foraminal stenosis, or nerve root/spinal cord compression at levels corresponding with clinical findings 1
  • Failed conservative therapy for at least 6 weeks must be documented, unless there are specific indications for waiver of conservative management, with a minimum of 6 weeks of conservative treatment required to be documented 1

Clinical Presentation Analysis

  • Symptoms consistent with cervical radiculopathy, such as neck pain radiating into arms, difficulty with balance, and loss of feeling in feet, can be an indication for ACDF when other criteria are met 2

Evidence Supporting ACDF When Criteria Are Met

  • ACDF is an established treatment for cervical radiculopathy with reported good to excellent outcomes in approximately 90% of patients when appropriate criteria are met 2
  • Studies have demonstrated that ACDF using allograft bone and plate stabilization can achieve fusion rates of up to 100% with good or excellent clinical outcomes in 83.3% of properly selected patients 3, 4
  • Allograft materials that are 100% bone are considered medically necessary for spinal fusions regardless of the shape of the implant, when other criteria for the procedure are met 1
