Treatment for Iron Deficiency Anemia in Females
Initial Diagnostic Workup
- The American Gastroenterological Association recommends checking for occult blood loss, especially from the gastrointestinal tract, as this is the most common cause of iron deficiency anemia in adult females, particularly post-menopausal 1, 2
- A thorough evaluation should be performed to identify the underlying cause of anemia, including drug exposure history, peripheral blood smear review, and assessment for iron, folate, and vitamin B12 deficiency 3
- Iron status should be assessed through serum iron, total iron-binding capacity (TIBC), transferrin saturation, and ferritin levels 3
- Coeliac disease should be considered as a potential cause of iron deficiency anemia, and ideally tested with tissue transglutaminase (tTG) antibody 2
Treatment Based on Underlying Cause
- The American College of Gastroenterology recommends initiating oral iron supplementation with ferrous sulfate 200 mg three times daily for iron deficiency anemia 3
- Iron therapy should be continued for three months after correction of anemia to replenish iron stores 3
- For patients with declining hemoglobin levels approaching 10 g/dL, treatment should be initiated promptly to prevent further decline 4, 5
Special Considerations
- The American Society of Clinical Oncology recommends considering erythropoiesis-stimulating agents (ESAs) if the anemia is related to chemotherapy and hemoglobin is ≤10 g/dL 4, 5, 6
- ESAs should not be initiated when hemoglobin is >10 g/dL due to increased risk of thromboembolic events 4, 5, 3
- The National Kidney Foundation recommends targeting hemoglobin levels between 10-12 g/dL for patients with chronic kidney disease 3
Red Blood Cell Transfusion
- The American Association of Blood Banks recommends considering RBC transfusion for severe symptomatic anemia or when hemoglobin falls below 7 g/dL in hemodynamically stable patients 3
- The minimum number of RBC units necessary should be used to relieve symptoms or return hemoglobin to a safe range 3
