Timeframe for Transfusing Single Donor Platelets After Blood Bank Issue

Critical Time Parameters

  • The Association of Anaesthetists recommends that infusion should be started within 30 minutes of removal from storage for all platelet products, as stated in their 2025 guidelines 1
  • The AAGBI guidelines from 2016 specify that transfusion should ideally be commenced within 30 minutes of removal from the platelet storage incubator 2

Transfusion Duration and Administration

  • Complete the transfusion within 30 minutes for a standard adult therapeutic dose, according to the 2016 AAGBI guidelines 2
  • Use a 170-200 μm filter through a standard blood administration set or dedicated platelet administration set, as recommended by the Association of Anaesthetists in 2025 and the AAGBI in 2016 1, 2
  • Never use a set that has already been used for red blood cells, as advised by the Association of Anaesthetists in 2025 and the AAGBI in 2016 1, 2

Storage Requirements Before Issue

  • Platelets are stored at 20-24°C with constant horizontal agitation in approved incubators, according to the Association of Anaesthetists in 2025 and the Journal of Clinical Oncology in 2001 1, 3, 4
  • Never refrigerate platelets, as this will destroy their viability, as stated by the AAGBI in 2016 2
  • Maximum storage from collection to transfusion is 5 days due to bacterial contamination risk, as recommended by the Association of Anaesthetists in 2025 and the Journal of Clinical Oncology in 2001 1, 3, 4, 5

Critical Pitfalls to Avoid

  • Temperature control is paramount, and the 30-minute window from removal to transfusion initiation minimizes the risk of bacterial proliferation while maintaining platelet function, as stated by the Association of Anaesthetists in 2025 and the AAGBI in 2016 1, 2
  • Do not confuse with red blood cell timing, as red blood cells have a 4-hour transfusion window after removal from refrigeration, but platelets require much faster administration, as advised by the Association of Anaesthetists in 2025 and the AAGBI in 2016 1, 2
  • Both single-donor apheresis platelets and pooled platelet concentrates from whole blood follow the same 30-minute rule and can be used interchangeably when leukoreduced, according to the Association of Anaesthetists in 2025 and the Journal of Clinical Oncology in 2018 1, 6

Expected Clinical Response

  • A standard adult therapeutic dose should increase the patient's platelet count by approximately 30 × 10⁹/L, as stated by the AAGBI in 2016 2
  • Recheck the platelet count after transfusion to assess response, as recommended by the AAGBI in 2016 2
