Acute Sinusitis Management with Corticosteroids

Indications for Oral Corticosteroids

  • The American Academy of Allergy, Asthma, and Immunology recommends oral corticosteroids for acute sinusitis only in specific situations, such as treatment failure after initial antibiotics and intranasal corticosteroids, marked mucosal edema, or presence of nasal polyps 1, 2, 3
  • Oral corticosteroids may be beneficial for patients with severe chronic hyperplastic sinusitis, particularly those with eosinophilic inflammation 3
  • The American College of Allergy, Asthma, and Immunology suggests that oral corticosteroids should only be used as short-term adjunctive therapy (3-7 days) in these specific situations 1

Treatment Algorithm

  • The Infectious Diseases Society of America recommends confirming bacterial sinusitis before prescribing antibiotics and considering adjunctive corticosteroids, using one of three patterns: persistent symptoms ≥10 days without improvement, severe symptoms (fever ≥39°C with purulent discharge) for ≥3 consecutive days, or "double sickening" - worsening after initial improvement 2, 3
  • The American Academy of Otolaryngology recommends starting with intranasal corticosteroids + antibiotics, with amoxicillin-clavulanate 875 mg/125 mg twice daily for 5-10 days as the first-line antibiotic 2

Special Considerations

  • The American Thoracic Society advises exercising caution when considering systemic corticosteroids in patients with asthma, immunocompromised patients, elderly patients, and patients with diabetes 2
  • The American Diabetes Association notes that oral corticosteroids may affect blood glucose control in patients with diabetes 2

Side Effects and Contraindications

  • The American Academy of Allergy, Asthma, and Immunology reports that short-course oral corticosteroids have a side effects profile that includes insomnia and mood changes, gastrointestinal disturbances, and temporary blood glucose elevation 1
