Type 1 Diabetes Management

Initial Treatment Approach

  • The American Diabetes Association recommends initiating intensive insulin therapy using either multiple daily injections (MDI) with 3-4 injections per day of basal and prandial insulin, or continuous subcutaneous insulin infusion (CSII/insulin pump), combined with insulin analogs to reduce hypoglycemia risk in newly diagnosed Type 1 Diabetes patients 1, 2, 3

Diagnostic Confirmation

  • Pancreatic autoantibodies testing (GAD, IA-2, ZnT8) is used to distinguish Type 1 DM from Type 2 DM, particularly in young adults with obesity where presentation may overlap 4, 5, 6
  • Assessment for diabetic ketoacidosis (DKA) is necessary, and if present, requires immediate IV insulin until acidosis resolves, then transition to subcutaneous insulin 4, 6, 7
  • Baseline HbA1c and fasting glucose measurements are used to establish severity 3

Initial Insulin Regimen

  • The initial total daily dose of insulin should be 0.5 units/kg/day for metabolically stable patients, split 50/50 between basal and prandial insulin 8
  • Rapid-acting insulin analogs (lispro, aspart, glulisine) should be used for prandial doses rather than regular human insulin to reduce hypoglycemia risk while achieving equivalent HbA1c reduction 1, 2, 3
  • Long-acting insulin analogs (glargine, detemir) should be used for basal coverage, administered once daily at bedtime, as they provide more stable glucose control with less hypoglycemia compared to NPH insulin 1

Multidisciplinary Team Approach

  • A multidisciplinary team including a physician, diabetes educator, registered dietitian, and mental health professional is necessary for comprehensive diabetes self-management education 4, 6

Monitoring Requirements

  • HbA1c measurement should be done every 3 months 3
  • Insulin regimen and insulin-taking behavior should be reassessed every 3-6 months 3
