Tretinoin Treatment Guidelines for Acne
Introduction to Tretinoin Therapy
- The American Academy of Dermatology recommends applying tretinoin once nightly as a thin layer to completely dry skin, starting with every-other-night application if skin is sensitive, and always combining with benzoyl peroxide in the morning as part of multimodal therapy—never using tretinoin as monotherapy 1, 2
Application and Dosing Strategy
- For mild acne, the guideline society recommends starting with adapalene 0.1% or tretinoin 0.025-0.05% once nightly, combined with benzoyl peroxide 2.5-5% gel applied in the morning 4, 2
- For moderate acne, use tretinoin 0.025-0.1% nightly plus benzoyl peroxide 2.5-5% in morning, adding topical antibiotic in fixed-dose combination with benzoyl peroxide if inflammatory lesions present 4, 2
Sun Protection and Maintenance
- Minimize sun exposure, use sunscreen and protective clothing when outdoors—tretinoin causes heightened photosensitivity 3
- After achieving clearance, continue tretinoin indefinitely to prevent recurrence, reducing to once weekly to three times weekly application for long-term maintenance 3
- Benzoyl peroxide can also be continued as maintenance 2
Contraindications and Precautions
- Pregnancy prevention is mandatory for persons of childbearing potential—tretinoin is pregnancy category C with no established safety in pregnant women 3
- Not established for use in children under 10 years of age 3
