Management of Neonatal Jaundice

Initial Assessment and Management

  • The American Academy of Pediatrics recommends obtaining comprehensive laboratory workup including total and direct bilirubin levels, blood type, direct antibody test, complete blood count, reticulocyte count, and serum albumin for newborns with jaundice 1, 3
  • The initial assessment should include evaluation of the infant's and mother's blood type to assess for potential ABO incompatibility 1, 3

Phototherapy Implementation

  • The American Academy of Pediatrics recommends initiating phototherapy according to gestational age and risk factor-based thresholds for newborns with jaundice 1, 4
  • Phototherapy should be implemented using special blue light in the 430-490 nm spectrum with irradiance of ≥30 μW/cm²/nm 4

Monitoring During Treatment

  • The American Academy of Pediatrics recommends continuing breastfeeding or bottle-feeding every 2-3 hours during phototherapy for newborns with jaundice 1, 4
  • Bilirubin levels should be monitored regularly, with repeat measurements every 4-6 hours for total serum bilirubin (TSB) < 20 mg/dL 1, 3

Discontinuation of Phototherapy and Follow-up

  • Phototherapy can be discontinued when serum bilirubin levels fall below 13-14 mg/dL 1, 3, 5
  • Follow-up bilirubin measurement should be obtained within 24 hours after discharge, especially for infants who received phototherapy before 3-4 days of age 4, 5
