Early Satiety: Differential Diagnosis and Treatment Approach

Differential Diagnosis

  • Gastroparesis is defined as delayed gastric emptying without mechanical obstruction, presenting with early satiety, postprandial fullness, nausea, vomiting, bloating, and abdominal pain, with the most common etiologies including diabetic gastroparesis (25% of cases), idiopathic gastroparesis, medication-induced, and post-surgical 1.
  • Functional dyspepsia presents with bothersome epigastric pain or burning, postprandial fullness, or early satiation without structural disease, and is classified into postprandial distress syndrome (PDS) and epigastric pain syndrome (EPS) 2.
  • Malignancy must be excluded first, particularly gastric or pancreatic cancer, which presents with early satiety and weight loss >10% 3.
  • Ovarian cancer can cause early satiety with advanced disease due to ascites and abdominal masses 4.

Diagnostic Approach

  • The American Gastroenterological Association recommends upper endoscopy to rule out mechanical obstruction before diagnosing functional or motility disorders 1.
  • Gastric emptying scintigraphy is the gold standard for diagnosing gastroparesis and must be performed for at least 2 hours, with 4-hour testing providing higher diagnostic yield 5.

Treatment Approach

  • The American College of Gastroenterology recommends dietary modifications, including small evening meals, small particle size, reduced fat diet, and calorie-dense liquid supplements, before considering pharmacotherapy 1, 3, 6.
  • Metoclopramide is the only FDA-approved medication for gastroparesis, but carries a high risk of extrapyramidal side effects, including irreversible tardive dyskinesia 1, 7.
  • Domperidone is preferred for extended therapy due to lower neurological risk, but carries a risk of QT prolongation, especially with doses >30 mg/day and in patients >60 years old 7.

Special Considerations

  • The American Heart Association recommends dietary modifications, including small evening meals, liquid meals, and low-FODMAP diet, for patients with cardiac amyloidosis and GI involvement 6.
  • The National Comprehensive Cancer Network recommends prokinetic agents, dietary modifications, and nutritional support for cancer patients with early satiety 5, 4, 7.
