Vascular Heel Ulcer Management

Critical First Steps: Assess Vascular Status

  • The American Diabetes Association recommends measuring ankle-brachial index (ABI) and ankle pressure immediately, and if ankle pressure is <50 mmHg or ABI <0.5, urgent vascular imaging and revascularization should be considered 2, 3
  • The International Working Group on the Diabetic Foot (IWGDF) suggests that if toe pressure is available, values <30 mmHg or TcPO2 <25 mmHg also warrant consideration for revascularization 4

Infection Management Despite Absent Clinical Signs

  • The Infectious Diseases Society of America recommends starting empiric oral antibiotic therapy targeted at S. aureus and streptococci (such as cephalexin, flucloxacillin, or clindamycin) 1, 2, 3
  • The IWGDF guidelines classify this as a mild infection requiring oral antibiotics, even though systemic signs are absent, and recommend obtaining wound culture from the debrided base to guide antibiotic adjustment 1, 2, 5

Local Wound Care Protocol

  • The Wound, Ostomy and Continence Nurses Society recommends performing sharp debridement with scalpel and repeating as frequently as needed (often weekly or more) 1, 2, 3
  • The European Wound Management Association suggests using alginates or foams to absorb purulent exudate and maintaining a moist wound environment 6, 1, 2

Pressure Offloading for Heel Ulcers

  • The American College of Foot and Ankle Surgeons recommends considering offloading with shoe modifications, temporary footwear, or orthoses for non-plantar ulcers like heel ulcers 1, 3, 8
  • The IWGDF suggests instructing the patient to limit standing and walking, use crutches if necessary, and ensuring heel protection to prevent pressure during bed rest 1, 3, 7

Additional Considerations

  • The Undersea and Hyperbaric Medical Society recommends considering hyperbaric oxygen treatment to hasten healing in poorly healing wounds 1, 2
  • The American Heart Association suggests emphasizing smoking cessation, control of hypertension and dyslipidemia, and use of antiplatelet therapy for cardiovascular risk reduction 2, 7, 4
