Vitamin B12 Deficiency Treatment and Recovery

Treatment Protocols

  • The American College of Physicians recommends administering hydroxocobalamin 1 mg intramuscularly on alternate days until no further improvement occurs, then transitioning to maintenance therapy, for patients with neurological involvement including memory impairment 1, 2
  • For deficiency without neurological involvement, the treatment guideline suggests giving hydroxocobalamin 1 mg intramuscularly three times weekly for 2 weeks 1

Monitoring and Maintenance

  • The clinical guideline recommends checking serum B12 and homocysteine every 3 months until stabilization, then annually, with a target homocysteine level <10 μmol/L for optimal neurological outcomes 1, 2
  • Monitoring for improvement in specific cognitive symptoms, such as difficulty concentrating, short-term memory loss, and "brain fog", is essential for assessing treatment efficacy 3

Safety Considerations

  • The medical guideline warns that folic acid should never be administered before treating B12 deficiency, as it may mask the deficiency while allowing irreversible neurological damage to progress 1, 2
