Apixaban and Corticosteroid Interaction

Mechanism of Action

  • Apixaban is metabolized primarily through CYP3A4 and is a substrate for P-glycoprotein transport, according to the American Heart Association 1, 2
  • Methylprednisolone does not significantly inhibit or induce CYP3A4 or P-gp, which are the two critical pathways that would alter apixaban exposure, as stated by the American Heart Association 1
  • The National Comprehensive Cancer Network guidelines specifically list contraindications for apixaban as strong dual inhibitors/inducers of CYP3A4 and P-gp, and corticosteroids are notably absent from this list, as recommended by the National Comprehensive Cancer Network 3, 4

Clinical Considerations

  • Corticosteroids can increase bleeding risk through gastric irritation and potential peptic ulcer formation, independent of any drug interaction, as noted by the American College of Gastroenterology 5
  • Short-term Medrol packs pose minimal additional bleeding risk compared to chronic corticosteroid use, according to the American College of Clinical Pharmacy 5
  • Consider adding proton pump inhibitor prophylaxis if the patient has additional risk factors, such as age >75 years, prior GI bleeding, concurrent NSAID use, or H. pylori infection, as recommended by the American Gastroenterological Association 5

Monitoring and Prevention

  • Counsel patients to watch for signs of bleeding, such as unusual bruising, blood in urine or stool, prolonged bleeding from cuts, or unexplained bleeding, as advised by the American College of Cardiology 5
  • Avoid concurrent NSAIDs during the Medrol pack, as NSAIDs significantly increase bleeding risk with apixaban, according to the American Heart Association 5
  • Use acetaminophen for additional pain relief if needed, as it does not increase bleeding risk with apixaban, as recommended by the American Pain Society 5
