IVIG Therapy for Autoimmune Necrotizing Myopathy

Clinical Presentation and Indication

  • The patient has autoimmune necrotizing myopathy with elevated CPK levels, indicating ongoing muscle inflammation despite prior treatment, which meets criteria for IVIG therapy 1
  • Elevated CPK is a marker of disease activity in inflammatory myopathies and indicates the need for continued immunomodulatory therapy 2
  • The patient's condition represents Grade 3 myositis, which according to guidelines warrants IVIG therapy when there is inadequate response to corticosteroids 3, 1

Treatment Protocol

  • The standard dosing for IVIG in inflammatory myopathies is 1-2 g/kg of ideal body weight, typically administered monthly 1
  • The prescribed 50g of Gammagard Liquid 10% is appropriate based on weight-based dosing recommendations for this condition 1
  • The scheduled treatment dates follow the appropriate monthly dosing schedule recommended for IVIG therapy in inflammatory myopathies 1

Premedication Regimen

  • Diphenhydramine 25mg is appropriate as premedication to prevent allergic reactions to IVIG 1
  • This premedication protocol represents standard practice for IVIG infusion to minimize adverse reactions 1

Monitoring and Follow-up

  • Regular monitoring of CPK levels is essential to assess treatment response, with decreasing levels indicating effective therapy 2
  • If symptoms and CPK levels do not improve after 4-6 weeks of IVIG therapy, consideration should be given to adding other immunosuppressants such as methotrexate, azathioprine, or mycophenolate mofetil 3
  • Patients should be monitored for potential adverse reactions to IVIG, including headache, infusion reactions, renal dysfunction, and aseptic meningitis syndrome 4

Common Pitfalls and Considerations

  • Failure to check IgA levels before administering IVIG can lead to severe anaphylactic reactions in IgA-deficient patients 1
  • IVIG should not be administered immediately before plasmapheresis, as it will be removed 5
