Cefuroxime Use in CKD for Post-Operative Antibiotic Prophylaxis

Guideline-Supported Use in CKD

  • The American College of Surgeons and other surgical prophylaxis guidelines recommend cefuroxime as an acceptable option for patients with renal impairment, with a standard prophylactic dose of 1.5g IV slow, single dose, for thoracic surgery, digestive surgery, orthopedic procedures, and vascular surgery 1, 2
  • For patients with penicillin allergy, cefuroxime serves as an alternative to cefazolin in multiple surgical contexts, demonstrating its safety profile even in compromised patients 1, 2, 3

Practical Dosing Algorithm

  • For prophylaxis in CKD patients with CrCl >20 ml/min, a standard dose of 1.5g IV slow, single dose, is recommended, with re-injection of 0.75g if surgery lasts more than 2 hours 1, 2

Safety Considerations Specific to CKD

  • The duration of cefuroxime prophylaxis must be limited to the operative period, with a maximum of 24 hours, to prevent drug accumulation and adverse effects 1, 2, 4

Common Pitfalls to Avoid

  • Patients with a history of immediate hypersensitivity to penicillins should not receive cefuroxime, due to the risk of cross-reactivity 4
  • If using cefuroxime for targeted prophylaxis, susceptibility should be confirmed by testing, especially in patients colonized with multidrug-resistant organisms 5
